Lake Superior Community Health Center (LSCHC) is a 501(c) 3 non-profit organization. Since its inception in 1972, LSCHC has grown and expanded, and today operates as a federally-qualified health center (FQHC), providing comprehensive primary and preventive health care services for the communities in the Twin Ports. We have clinic locations in Duluth, MN and Superior, WI. We treat the whole family in an integrated care model, regardless of insurance or income.


Timeline of Key Events

  • 1972

    Duluth Community Health Center (DCHC) opens as a free clinic in the basement of Sacred Heart Church in Duluth’s Central Hillside and becomes incorporated.

  • 1973

    The clinic relocates to a former grocery store at 2 E. 5th Street.

  • 1991

    The Health Care Access Office opens.

  • 1993

    The DCHC becomes a Federally-Qualified Health Center (FQHC).

  • 2000

    The clinic first receives federal Section 330 grant funding.
    Services are initiated in Superior at 1419 Hill Avenue. The clinic changes its name to Lake Superior Community Health Center (LSCHC).

  • 2001

    In 2001, a total of 25 FTE staff served 3,296 patients and provided 7,345 visits. The total agency budget was $1.6 million.

  • 2005

    The clinic relocates to its current location at 4325 Grand Avenue in Duluth. The Superior clinic relocates to 3600 Tower Avenue, and dental services are initiated.

  • 2007

    Dental services initiated in Duluth.

  • 2009

    Dental services expanded in Duluth.

  • 2011

    In 2011, a total of 74 FTE staff served 10,873 patients and provided 31,858 visits. The total agency budget was $6.5 million.

  • 2013

    LSCHC became Minnesota Department of Health "Health Care Home Certified" at our Duluth and Superior clinic locations. We have remained certified ever since. This certification increased our ability to provide care coordination and collaboration between primary care clinicians and community resources to support whole person care. This has improved the quality, experience, and value of care our patients receive.

  • 2018

    LSCHC a third clinic site in Superior, WI to offer outpatient substance abuse services and additional behavioral health services. The clinic is located at 1500 No 34th St, Suite 200.

  • 2020

    On August 4th, 2020, with the purchase of a new building in Carlton, MN, LSCHC opened a new dental practice to serve Carlton County and the surrounding area.

All Updates

This health center receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
