Do I need to make an appointment?
It is recommended that you make an appointment for medical services, but you may call the
clinic to see if there are any available walk-in appointments for that
day. Sometimes people do not show up for their appointments, so you
are welcome to come in and wait and we will do our very best to get
you in for an appointment. If it is an emergency, please call 911, go
to the local Urgent Care Center or the Emergency Room. The SMDC Nurse-On-Line
# is 786-3110 or 1-800-714-4646.
You must call to schedule an appointment for dental services.
What if I need to cancel or change the appointment?
Our goal is to provide timely access to care for all patients. You must call us at least 24 hours in advance if you’re unable to make it to your appointment on time.
Any patient who is late for or does not show up for their scheduled appointment and fails to call to cancel it at least 24 hours in advance may be placed on inactive patient status.
What do I bring to my appointment?
- If you have health or dental insurance, please bring the insurance card.
- ONE item for proof of income such as: a recent check stub, the
most recent W2 forms from your taxes, Form 1040 or a letter from Social
Security. Proof of income is required at the beginning of every year.
- A current list of your medications or the bottles of your medications.
- If your children are being seen at the health clinic, please bring immunization
records.
